  Library Card Application

 

To apply for a card, simply come to the library and fill out an

 

application. Applicants must be at least 18 years old and have a

 

phone number. Please bring your valid Photo ID, valid

 

Driver's License, or valid Federal Photo ID. If current address is

 

different from Photo ID please provide current service bill

 

with your current address. Children must be at least 5 years old

 

and be accompanied by a parent or guardian to apply. Out of

 

county residents can purchase a one year card for $20.00. Cards

 

are issued without cost to Monroe County residents. In order to

 

check out materials, cardholders must be present to hand their

 

cards to the librarian. No one else is permitted to use your card.

 

Replacement cards cost $3.00 each. Please report a missing card

 

immediately because you are responsible for all materials

 

checked out on your card!
 

The first two times a new card is used, the card holder will be

 

limited to two materials at check out. Multi-disc series dvd’s or

 

Non-traditional items cannot be checked out by new patrons. On

 

the third check out, a patron can check out up to 15 items.
